Hi can anyone recommend someone that makes custom moderators please.

I've never heard of anyone that would make you a custom moderator. Unless by "custom" you mean simply putting your name or logo on it. The R&D process is too long and drawn out for most manufacturers to do a 1-off.
I would start by looking for someone with a 3D printer. Also, you might look at Thingiverse and see if the design you've got hasn't already been done by someone else:
